Analysis of Medical Service Use for Cerebrovascular Diseases

Abstract
Purpose of the research£ºThis research aims to describe medical service use by patients with cerebrovascular diseases (CVD) and provide basic data for policy to control CVD on a national level.

Research materials and methods : Medical Service Benefit Data from the National Heath Insurance Corporation during from year 2000 to 2004 were employed to describe medical expenses by those who received a treatment for CVD and the number of patients, prevalence rates, number of medical service, days of hospital visit in terms of by year, sex, age, area, types of service provider, season, and socio-economic status.

Research result£ºThe research result includes the following. 1) The number of patients with CVD who received medical service increased to 52.9% from 350,660 persons in 2000 to 536,888 persons in 2004. The medical expense for CVD increased by 50.0% from 354.2 billion won in 2000 to 531.2 billion won in 2004, which is 1.5 times higher than that of medical expense for the total diseases. The average annual medical expense for CVD per person was much higher than that for high blood pressure disease, diabetes, and ischemic heart diseases. 2) The ratio between male patients with CVD and female was 48:52 as of 2004, which means there were more female patients. Yet, the medical expense was 50:50 between them. For sex-specific prevalence rates, 10.5 for men and 11.8 for women in 2004. 3) As for the portion of patients with CVD by age, 36.2% of the total 536,888 patients with CVD was those in their 70s or over, which is the highest portion. Also, those in their teens or below occupied 0.7% of the entire portion. Also for the prevalence rate of CVD per 1,000 population by age, those in their 70s or over were proven the highest prevalence rate. 4) As for the prevalence rate of CVD by socio- economic status, there were 39.2 patients per 1,000 population among those under medical benefit scheme and 10.0 persons holding health insurance in 2004. The average day of hospital visits and the average medical expense per person were 27.4 days and 1,620,000 won for those under medical benefit scheme, and 13.4 days and 910,000 won for those holding health insurance in 2004. The average day of hospital visits and medical expense per person by the level of the payment of health insurance premiums, was the greatest for those in the upper 10% of people who pay higher premiums, and the average annual increase ratio was also the highest with them.

Conclusion£ºA tendency of increase was found in numbers of patients and medical expenses for those with cerebrovascular diseases during from year 2000 to year 2004. Increase rate of medical expenses for CVD was higher than that for other chronic diseases. This pattern of change varies across sociodemographic groups.
